You have to configure X so that you have a second display. Standard display 
is :0.0, your second one (TV) will be :0.1
I'm using the following alias in my ~/.bashrc:

alias mptv="/usr/local/bin/mplayer -fs -fixed-vo -display :0.1 "

All you have to do then is "mptv mymovie.avi". Works great :)
